Collagen: The Invisible Structure that Supports Your Skin
Collagen is the most abundant protein in your body —and, within your face, it's what keeps the skin firm, smooth, and well-supported. It's the invisible structure upon which everything else rests. When it starts to fail, you don't notice it in a specific area: you notice it as a whole. The skin looks thinner, less elastic, more tired. It's not a wrinkle, it's not a spot. It's something that says “I'm no longer at my best”.
The good news is that collagen can be stimulated. Not by completely stopping aging —that doesn't exist— but by reactivating your own production with well-designed medical treatments.
At Clínicas Santé, on Avenida Diagonal 384 in Barcelona, we work with several protocols specifically focused on recovering firmness and stimulating collagen. We explain how.
Why We Lose Collagen with Age
From the age of 25, we lose approximately 1% of collagen per year. This decline accelerates during perimenopause and menopause in women (with an estimated peak loss of around 30% in the first years) and more progressively in men.
The causes are a combination of:
- Natural reduction in fibroblast activity (the cells that produce collagen).
- Hormonal decline, especially estrogen.
- Oxidative stress accumulated from UV radiation, pollution, smoking, alcohol.
- Glycation (dietary sugar hardens and degrades collagen fibers).
- Genetics.
The visible result: thinner skin, progressive sagging, fine lines, less elasticity, less defined facial contours.
Do Creams and Supplements Work?
A question we receive daily. The honest answer:
- Creams with collagen do not provide functional collagen to the dermis; the molecules are too large to penetrate.
- Retinoids, vitamin C, and peptides can stimulate collagen synthesis when used at appropriate concentrations and for the necessary time.
- Oral collagen peptide supplements have reasonable evidence of effect on skin and joints, but their impact is modest.
- Diet and lifestyle are fundamental: quality protein, vitamin C, sleep, no smoking, daily sun protection.
Cosmetics and habits do part of the work. But when we talk about actively stimulating collagen synthesis in the dermis, medical treatments offer a different level.
Medical Treatments to Stimulate Collagen
1. HIFU (High-Intensity Focused Ultrasound)
HIFU works with focused ultrasound energy that creates small, controlled heating points in the deep dermis and fascia. This tiny thermal injury triggers a repair response that produces new collagen over the following months. It is the most used treatment at Santé for moderate sagging of the facial oval, neck, and submentum. A single session, results that build over 3–6 months, lasting 12–18 months.
2. Injectable Biostimulators
Biostimulators are injectable products specifically designed to stimulate collagen:
- Sculptra (poly-L-lactic acid): slowly reabsorbs while stimulating type I collagen. Very progressive results over 3–6 months, lasting up to 24 months. Indicated for overall firmness loss.
- Radiesse (calcium hydroxyapatite): combines immediate volumetric effect
