The double chin is one of the signs of aging that most concerns our patients — and one of the most difficult to treat with superficial cosmetic methods. Neck creams, jaw exercises, massages: none address the real cause of the problem, which lies in the deep layers of the tissue.

Submental HIFU — High-Intensity Focused Ultrasound applied to the double chin area — is the non-surgical treatment with the greatest ability to act on these deep layers: the platysma (neck muscle that contributes to tissue sagging) and the submental skin with loss of elasticity.

It's not magic and it doesn't replace surgery in severe cases. But for moderate-sized double chins with skin and platysma laxity, HIFU is currently the most effective non-invasive treatment available.

Anatomy of the Double Chin: Why Understanding What's Inside Matters

Beneath the skin of the double chin are several layers that contribute to the problem:

  1. Skin: may be lax, with loss of elasticity and collagen
  2. Submental fat: accumulation of adipose tissue (variable by patient)
  3. Platysma muscle: the neck muscle that, when it loses tone, sags forward and downward, accentuating the double chin
  4. Cervical fascia: the covering of the neck that, when it loses tension, gives that "sagging" appearance to the neck

HIFU can act on all these elements except fat (lipolytic enzymes are used for fat). But its greatest value lies in tightening the platysma and skin — the components that define the "quality" of the result.

How HIFU Acts on the Double Chin

HIFU transducers emit converging ultrasound beams that are focused at a precise depth. In the submental area, we primarily use two depths:

4.5 mm: platysma and cervical fascia level. The energy generates heat points (65-70°C) that:

  • Contract platysma fibers at the moment of the shot
  • Stimulate the production of new collagen in the fascia
  • Create a mechanical tightening effect of the deep tissue

3 mm: subcutaneous cellular tissue and deep dermis level:

  • Tightening of the hypodermis
  • Initiation of neocollagenesis in the deep dermis

1.5 mm: mid and superficial dermis:

  • Improvement of skin quality
  • Reduction of fine lines on the neck

What Results Does HIFU Produce on the Double Chin?

Immediate Results

Immediately after the session, there is a visible contraction of the tissue — existing collagen has contracted due to heat. The neck appears somewhat tighter. This immediate effect is real but less significant than the final result.

Delayed Results (2-6 months)

The main result of HIFU appears over the following months, as new collagen forms and matures. At 3-6 months, typical results in the submental area include:

  • Visible tightening of the neck skin and double chin
  • Lifting of the submental tissue
  • Improved jawline definition
  • Reduction of the "curtain" of skin that sags from the neck
  • Improvement in the texture and quality of the neck skin

Duration

The Treatment at Santé

Prior Assessment

The doctor evaluates the composition of the double chin (fat, skin laxity, platysma tone), the degree of involvement, and expectations. They determine if HIFU is sufficient or if a combined protocol (enzymes + HIFU) is more appropriate.

The Session

Conductive gel is applied to the submental and neck area. The doctor works with 4.5 mm, 3 mm, and 1.5 mm cartridges according to the protocol. The session lasts between 20 and 35 minutes for the submental area and neck.

The sensation during treatment is one of intense, brief heat with each shot. Tolerability varies — some patients find it uncomfortable. The doctor may recommend ibuprofen 30-45 minutes beforehand if sensitivity is high.

Post-Treatment

There may be:

  • Mild redness in the first few hours
  • Some neck sensitivity for 24-48 hours
  • Occasionally, small pinpoint bruises

There is no real downtime. The patient can resume normal activities on the same day.
