Sculptra, Radiesse, and Lanluma: Differences Among the Three Major Biostimulators

Three biostimulators, one principle: stimulating the body

Collagen biostimulators have transformed aesthetic medicine because they solve a problem that hyaluronic acid cannot: the progressive loss of tissue architecture. They do not fill the empty space with an external gel; they stimulate the patient's own fibroblasts to produce new collagen, improving the quality, density, and firmness of the tissue from within.

The three names most frequently mentioned in consultations are Sculptra, Radiesse, and Lanluma. All three are biostimulators, but their composition, mechanism, indications, and results are different. Understanding these differences is fundamental to choosing the most suitable one for each person and each area.

In this article, the medical team at Santé Clínica Estética explains the key aspects of each.

Sculptra: the pioneer of facial biostimulation

Composition and mechanism

Sculptra contains microparticles of poly-L-lactic acid (PLLA) suspended in sterile water. PLLA is a biodegradable polymer that the body is familiar with: it has been used in absorbable surgical sutures for decades.

When injected into the facial subdermal tissue, the microparticles trigger a controlled inflammatory response that activates fibroblasts. These produce new type I and III collagen during the 2-6 months following the injection. The PLLA particles degrade slowly, resulting in lasting neocollagenesis: new tissue belonging to the patient.

Application areas

Sculptra is especially indicated for:

  • Loss of malar and temporal volume (cheekbones, sunken temples)
  • Deep nasolabial folds and marionette lines
  • Overall improvement of tissue quality in patients over 40 years old
  • Facial atrophy associated with weight loss or aging

Results and duration

The results are not immediate. Improvement is progressively observed during the 3-6 months post-treatment. The duration is 18-24 months depending on individual response. Typically, 2-3 sessions spaced 6-8 weeks apart are required.

Radiesse: biostimulator with immediate effect and structural remodeling

Composition and mechanism

Radiesse is composed of calcium hydroxylapatite (CaHA) microspheres suspended in a carboxymethylcellulose carrier gel. Calcium hydroxylapatite is the main mineral in human bone — a biocompatible component with which the body has a known relationship.

Radiesse has a dual mechanism of action:

  1. Immediate effect: the carrier gel acts as a volume filler at the time of injection, providing a visible result from the first session.
  2. Progressive biostimulation: CaHA microspheres stimulate collagen production over the following months, while the carrier gel is gradually reabsorbed.

This combination makes Radiesse a product with a different result curve than Sculptra: the patient sees immediate changes, which consolidate and naturalize over time.

Application areas

Radiesse is particularly useful in:

  • Jawline definition and facial oval firming
  • Hand rejuvenation (an area of high sun exposure and rapid aging)
  • Deep nasolabial folds and sagging cheeks
  • Correction of projection loss in cheekbones
  • Neck and décolletage area (in hyperdiluted solutions)

The so-called hyperdiluted Radiesse technique (with a higher proportion of saline solution) is used to treat large surfaces such as the neck, décolletage, or arms, where the goal is to improve skin quality rather than add volume.

Results and duration

The duration of Radiesse is 12-18 months for the volume effect, and up to 24 months for the biostimulating component. Generally, 1-2 sessions are needed for the initial goal.

Lanluma: high-volume biostimulation for the body

Composition and mechanism

Lanluma, like Sculptra, contains poly-L-lactic acid (PLLA), but with a different formulation: the particles are larger and the concentration is optimized to treat large body volumes. It has been specifically developed and approved for areas such as the buttocks, thighs, and lumbar region, where Sculptra and Radiesse (in their standard formulations) are not designed to operate.

The mechanism of action is the same: fibroblast stimulation and neocollagenesis. But the scale of treatment is different: Lanluma can work with large, deep subcutaneous surfaces safely and effectively.

Application areas

Lanluma is indicated for:

  • Buttocks: volume augmentation, improvement of firmness and definition
  • Inner and outer thighs: tightening of loose skin
  • Lumbar region and flanks: body remodeling
  • Arms: improvement of flaccidity on the inner arm

Results and duration

The results are similar to Sculptra in terms of progressiveness (2-6 months) and duration (18-24 months). Typically, 2-4 sessions are required for large body areas.

Comparative table: Sculptra vs Radiesse vs Lanluma

| | Sculptra | Radiesse | Lanluma |

|---|---|---|---|

| Active component | PLLA | CaHA | PLLA (body formulation) |

| Immediate effect | No | Yes (carrier gel) | No |

| Biostimulation | Yes | Yes | Yes |

| Main indication | Face (volume loss) | Face + hands + neck | Body (buttocks, thighs) |

| Duration | 18-24 months | 12-24 months | 18-24 months |

| Usual sessions | 2-3 | 1-2 | 2-4 |

Which one is right for you? The decision is medical

The choice between Sculptra, Radiesse, and Lanluma is not a matter of personal preference: it is a medical decision. It depends on:

  • The area to be treated and its anatomical characteristics
  • The degree of volume loss or tissue quality
  • The patient's clinical profile (age, history, expectations)
  • The type of result sought (immediate vs. progressive)
  • The possibility of combining biostimulators with other treatments

At Santé Clínica Estética, we perform a detailed medical assessment before any protocol with biostimulators. There is no one product that is better than another in the abstract: there is the most suitable product for that patient at that time.

Frequently Asked Questions

Can Sculptra, Radiesse, and Lanluma be combined in the same treatment?

In some cases, yes. Combined protocols are possible and, in patients with both facial and body volume loss, can be designed sequentially. Always under medical discretion.

Are all three safe?

All three have a well-documented safety profile with decades of clinical use. The most common side effects are bruising, local edema, and, in the case of PLLA, possible nodule formation if the indicated post-treatment massage is not performed.

How long do the results last?

Between 18 and 24 months in all three cases, with individual variations depending on the response to treatment, lifestyle, and the treated area.

Are the results reversible?

Not directly: as biostimulators, the generated collagen is the patient's own tissue. It cannot be removed in the same way hyaluronic acid can be dissolved. This is why a thorough prior medical indication is essential.
