Double chin: a problem no longer monopolized by surgery

For years, the double chin (excess submental tissue — fat, loose skin, or both) was a problem that only surgery could effectively resolve: submental liposuction or cervicofacial lift. These treatments are effective, but they involve everything that a surgical intervention entails: general anesthesia, scars, prolonged recovery time, and risks inherent to the surgical act.

Modern aesthetic medicine now offers a real and effective alternative for most cases of moderate double chin. It is not a single technique: it is a combined, personalized, and sequenced protocol that attacks the problem from all angles — fat, loose skin, lack of jawline definition, and excess tissue.

At Santé Clínica Estética, we design these protocols with a rigorous medical approach, adapting the techniques to the specific type of double chin of each patient.

Why the double chin is not "just one problem"

Before discussing treatments, it is important to understand the composition of the problem. Submental double chin can be:

  • Submental fat: accumulation of adipose tissue in the area under the chin
  • Loose skin: excess skin due to loss of elasticity (aging, rapid weight loss)
  • Platysma muscle hypertrophy: the neck muscle which, when it loses tone, contributes to tissue sagging
  • Lack of mandibular projection: in patients with a retrognathic (recessed) jaw, the double chin appears more pronounced because the jawline is not well defined
  • Combination of several factors: the most common scenario

The appropriate protocol depends on which factors predominate in each case. A protocol designed only to reduce fat in a patient whose main problem is loose skin will not yield the expected results.

The four pillars of the combined protocol

Pillar 1: Lipolytic enzymes — reduction of submental fat

Lipolytic enzymes (also called phosphatidylcholine or localized fat lysis products) are injectables that act on the membranes of fat cells, causing their rupture and the elimination of lipid content via the lymphatic system.

For a fatty double chin, they are the most direct tool: they target the fatty component locally, with progressive results over 4-8 weeks post-treatment.

  • Number of sessions: usually 2-3, spaced 4-6 weeks apart
  • Post-treatment inflammation: normal and expected — indicates that the lysis process is occurring
  • Result: permanent reduction of fat cells in the treated area

Pillar 2: HIFU (High-Intensity Focused Ultrasound) — tissue and skin tightening

HIFU is the most effective non-invasive treatment for tightening the platysma and submental neck skin. It acts on deep layers (SMAS, fascia), heating the tissue to temperatures that cause immediate contraction of collagen fibers and progressive neocollagenesis over several months.

For the double chin, HIFU:

  • Tightens the platysma and skin of the submental neck
  • Lifts sagging tissue
  • Improves skin quality in the treated area
  • Enhances the results of enzymes: after fat reduction, HIFU tightens the skin that has less support

HIFU is usually applied before or after enzymes, with an interval of 4-6 weeks.

Pillar 3: Mesotherapy — improving skin quality and local metabolism

Mesotherapy in the submental area can incorporate:

  • Mild lipolytics: caffeine, L-carnitine, umbilical tissue extract — which complement the action of enzymes in the most superficial part of the adipose tissue
  • Vitamins and amino acids: improve the quality of the neck skin, an area often neglected in skincare routines
  • Low molecular weight hyaluronic acid: intradermal hydration that improves the elasticity and texture of the neck skin

Mesotherapy is incorporated into the protocol when there is a skin quality component or when local metabolism needs to be improved between enzyme and HIFU sessions.

Pillar 4: Hyaluronic acid — jawline definition

The most underestimated component in double chin treatment: jawline contouring with hyaluronic acid.

In many patients, the double chin appears larger than it actually is because the jaw lacks sufficient definition. By defining the mandibular angle and jawline with hyaluronic acid, the transition between the face and neck becomes cleaner and more visible, which "visually reduces" the double chin even without modifying the submental tissue.

Jawline contouring is usually performed as the last step of the protocol, once the results of the other treatments have been obtained, to fine-tune the final definition.

How we design the protocol at Santé

The first session is always a complete medical assessment:

  1. Analysis of the double chin's composition: predominant fat, loose skin, muscle component, retrognathia
  2. Expectation evaluation: what realistic results can be achieved in each case
  3. Protocol design: sequence of treatments, intervals, number of sessions for each
  4. Review of contraindications: anticoagulants, history of surgery in the area, active inflammation

With this information, we design the specific protocol: not all patients need all four pillars. In some cases, two or three techniques are sufficient.

Typical protocol sequence

| Week | Treatment |

|--------|-------------|

| 0 | Medical assessment + first enzyme session |

| 4-6 | Second enzyme session (if indicated) |

| 6-8 | Submental HIFU |

| 8-10 | Partial results assessment |

| 10-12 | Reinforcement mesotherapy (if indicated) |

| 14-16 | Jawline contouring with HA (if indicated) |

| 16-20 | Final results review |

This sequence is indicative and adapted according to each patient's response.

Expected results

With a well-indicated complete protocol:

  • Visible reduction of submental fat (in patients with a predominant fatty component)
  • Tightening of the neck skin and platysma
  • Improved jawline definition
  • Perceptual reduction of the double chin due to the combination of less fat, more tension, and better jawline definition

The results are not equivalent to surgery in very severe cases (large double chins with a lot of loose skin), but in moderate cases, they are very significant and with much less recovery time.

Who it is for and who it is not for

Indicated for:

  • Moderate-sized double chins, predominantly fatty or mixed
  • Patients who do not want or cannot undergo surgery
  • Cases of moderate loose skin after weight loss
  • Patients with an ill-defined jawline that contributes to the perception of a double chin

Less suitable for:

  • Very large double chins with significant excess skin (require surgery for excess skin)
  • Patients with expectations of surgical results in minimal surgical time

Frequently asked questions

How much recovery time do I need?

Enzymes cause inflammation for 3-7 days — this is normal and expected. HIFU has minimal recovery. Overall, the patient can maintain normal activity with mild discomfort during the first weeks of treatment.

Are the results permanent?

Fat reduction from enzymes is long-lasting (fat cells do not regenerate). HIFU tightening lasts 12-18 months. Hyaluronic acid, 12-18 months. Maintenance also depends on lifestyle.

How much does the complete protocol cost?

The price varies depending on the treatments included and the number of sessions for each. The doctor will present the personalized protocol with cost details during the consultation.

Is it compatible with diet and exercise?

Yes. Medical treatments and healthy lifestyle habits are complementary — results are enhanced with both.
