Collagen: The Invisible Structure Supporting Your Skin
Collagen is the most abundant protein in your body — and, in your face, it’s what keeps your skin firm, smooth, and well-supported. It’s the invisible structure everything else relies on. When it starts to fail, you don’t notice it in one specific area: you notice it generally. Your skin looks thinner, less elastic, more tired. It’s not a wrinkle, it’s not a spot. It’s something that says “I’m not at my best anymore.”
The good news is that collagen can be stimulated. Not by completely stopping aging — that doesn’t exist — but by reactivating your body's own production with well-designed medical treatments.
At Santé Clinics, on Avenida Diagonal 384 in Barcelona, we offer several protocols specifically focused on restoring firmness and stimulating collagen. We’ll explain how.
Why We Lose Collagen with Age
From the age of 25, we lose approximately 1% of collagen per year. This decline accelerates during perimenopause and menopause in women (with an estimated peak loss of around 30% in the first few years) and more progressively in men.
The causes are a combination of:
- Natural reduction in fibroblast activity (the cells that produce collagen).
- Hormonal decline, especially estrogen.
- Oxidative stress accumulated from UV radiation, pollution, smoking, alcohol.
- Glycation (dietary sugar hardens and degrades collagen fibers).
- Genetics.
The visible result: thinner skin, progressive sagging, fine lines, less elasticity, less defined facial contours.
Do Creams and Supplements Work?
A question we receive daily. The honest answer:
- Collagen creams do not provide functional collagen to the dermis; the molecules are too large to penetrate.
- Retinoids, vitamin C, and peptides can stimulate collagen synthesis when used at appropriate concentrations and for the necessary duration.
- Oral collagen peptide supplements have reasonable evidence of effect on skin and joints, but their impact is modest.
- Diet and lifestyle are fundamental: quality protein, vitamin C, sleep, no smoking, daily sun protection.
Cosmetics and habits do part of the work. But when it comes to actively stimulating collagen synthesis in the dermis, medical treatments offer a different level.
Medical Treatments to Stimulate Collagen
1. HIFU (High-Intensity Focused Ultrasound)
HIFU uses focused ultrasound energy to create small, controlled heating points in the deep dermis and fascia. This tiny thermal injury triggers a repair response that produces new collagen over the following months. It is the most common treatment at Santé for moderate sagging of the jawline, neck, and submentum. A single session, results build over 3–6 months, lasting 12–18 months.
2. Injectable Biostimulators
Biostimulators are injectable products specifically designed to stimulate collagen:
- Sculptra (poly-L-lactic acid): slowly reabsorbs while stimulating type I collagen. Very progressive results over 3–6 months, lasting up to 24 months. Indicated for overall loss of firmness.
- Radiesse (calcium hydroxylapatite): combines an immediate volumetric effect with medium-term biostimulation. Very useful for the jawline, cheeks, and hands.
- Polynucleotides: purified DNA fragments that stimulate fibroblasts and improve skin quality. Especially useful for thin skin and sun damage.
3. Microneedling and Radiofrequency Microneedling
Radiofrequency microneedling combines micro-needles that create channels in the dermis with radiofrequency energy. The skin responds with collagen production and reorganization of the dermal matrix.
4. Plasma Gel and PRP
PRP (platelet-rich plasma) and plasma gel — one of our most distinctive offerings — use growth factors from your own plasma to stimulate fibroblasts and promote tissue regeneration.
5. Mesotherapy with Vitamins
Mesotherapy with vitamins directly delivers to the dermis the nutrients that fibroblasts need to produce collagen (vitamin C, precursor amino acids, antioxidants).
6. Fractional Laser
Fractional lasers generate micro-columns of heat that activate the regenerative cascade of collagen, especially useful when there is also sun damage and uneven texture.
Indications
These treatments are indicated for:
- Patients from 30 years old with preventive criteria.
- Skin that is progressively losing firmness.
- Incipient or moderate sagging of the facial oval.
- Women in perimenopause and menopause.
- Men with tired skin and loss of smoothness.
- Significant post-weight loss.
Specific contraindications are reviewed on a case-by-case basis during consultation.
How We Design a Protocol at Santé
Typically, it’s not a single treatment but a combined, phased plan. For example:
- Phase 1 (month 0–1): mesotherapy with vitamins + HIFU for hydration, radiance, and structural firmness.
- Phase 2 (month 1–3): injectable biostimulator (Sculptra or Radiesse) to initiate medium-term collagen production.
- Phase 3 (month 3–6): reviews, touch-ups, possibly polynucleotides or maintenance sessions.
Afterward, spaced maintenance according to individual response.
Expected Results
Results are progressive: collagen is not produced in a week. You should expect 3–6 months to see the full effect of a stimulating treatment.
- After HIFU: visible firmness at 2–3 months, maximum at 6 months.
- After biostimulators: progressive improvement in 3–6 months.
- After polynucleotides: improved skin quality in 4–8 weeks after a complete protocol.
What you'll notice: firmer skin, more defined jawline, better skin quality, more supported facial contours. Without radical transformation, with natural results.
Frequently Asked Questions
When is it advisable to start “working on collagen”?
From age 30 with gentle preventive measures. From age 40, protocols can be more active.
Is biostimulation the same as fillers?
No. Classic fillers (hyaluronic acid) provide physical volume. Biostimulators stimulate your own collagen — the effect builds over time, and when the product reabsorbs, your own collagen remains.
How many sessions are needed?
It depends on the treatment. HIFU: 1 session, annual maintenance. Sculptra: 2–3 sessions spaced 4–6 weeks. Polynucleotides: a protocol of 2–4 sessions.
Are there risks?
Low in qualified medical hands. We review individual contraindications during consultation. Common side effects are mild and transient.
Will people notice a change in me?
No. Collagen stimulation produces an effect of “you look more rested / fresh,” never a transformation.
Do oral supplements and diet work?
As a complement, yes. As a substitute for medical treatments, no.
