Why HIFU is Different From Everything Before
When a patient comes to the consultation with the question "what can I do to tighten my face without surgery?", the answer with the greatest scientific and clinical backing is HIFU: High-Intensity Focused Ultrasound.
It's not radiofrequency (which primarily acts on the dermis). It's not laser (which acts on the epidermis or superficial dermis). HIFU acts on layers that surface treatments cannot reach: the deep subcutaneous tissue, the fascia and — fundamentally — the SMAS (Superficial Musculoaponeurotic System): the same structure that surgeons address in a surgical lift.
That is the key difference: HIFU treats the structural cause of tissue sagging, not just the surface.
What is the SMAS and Why it Matters
The SMAS is a fibrous network that connects facial muscles to the skin. It is the "scaffolding" of facial tissue. When the SMAS loses tension — due to aging, gravity, collagen loss — the overlying tissue sags: jowls, more pronounced nasolabial folds, a jawline that loses definition.
A surgical lift acts on the SMAS physically, tightening it and suturing it into a new position. HIFU acts on the SMAS thermally: it generates focused heat points at the exact depth of the SMAS that contract the fibers and stimulate the production of new collagen.
The result is similar in principle to a surgical procedure — SMAS tightening — but without surgery, without scars, and with minimal recovery time.
How HIFU Works: The Physics Behind the Result
HIFU emits ultrasound beams that converge at a precise focal point at predetermined depths. The energy is concentrated at this focal point until it reaches temperatures of 65-70°C locally, without damaging the skin or intermediate tissues.
This temperature causes two simultaneous effects:
- Immediate collagen contraction: heat denatures existing collagen fibers, which contract at the time of treatment. Result: immediate tissue tightening.
- Delayed neocollagenesis: controlled thermal damage triggers a regenerative response that produces new collagen over the following 3-6 months. Result: progressive tightening and improvement in tissue quality.
HIFU cartridges allow treatment at different depths:
- 4.5 mm: SMAS level — maximum lifting effect
- 3 mm: subcutaneous tissue — hypodermis tightening
- 1.5 mm: deep dermis — improved texture and superficial tension
The protocol at Santé combines multiple depths in each session for a complete treatment.
At Santé: Ultraformer III Technology
At Santé Clínica Estética, we work with the Ultraformer III, one of the most advanced HIFU devices on the market. Its advantages over previous generations:
- Faster firing speed: the treatment is more comfortable because the shots are faster and more controlled
- Higher density cartridges: more energy points per session without needing to increase time
- Body transducers: allows HIFU to be applied to body areas (abdomen, thighs, arms) in addition to the facial area
- Feedback screen: the doctor visualizes the depth and energy applied in real time
Equipment quality matters: two clinics that claim to offer "HIFU" may be working with very different technologies in terms of results and safety.
Indications for Facial HIFU at Santé
Face:
- Facial tissue sagging and loss of jawline definition
- Jowls (lateral jowls or bulges at the mandibular angle)
- Nasolabial folds deepened by tissue sagging
- Cheek laxity
- Drooping forehead and upper eyelid (brow lift effect)
- Lax neck
Neck and double chin:
- Excess submental tissue (double chin)
- Neck with lax skin
Body areas:
- Post-partum or post-weight loss abdomen
- Inner thigh
- Inner arm
- Knees
Who is it for and Who is it Not for
Ideal for:
- Patients between 35 and 65 years old with moderate to moderate-severe skin laxity
- People who want to delay or avoid surgery
- Post-surgical maintenance (HIFU extends results)
- Younger patients with early loss of skin tension
Less suitable for:
- Very severe laxity with abundant excess skin (surgery yields better results)
- Patients with metal implants in the area (pacemakers, metal clips)
- Active areas of inflammation or infection
The Treatment at Santé: What the Session is Like
Prior Assessment
The doctor evaluates the degree of laxity, the areas to be treated, skin type, and objectives. They determine which cartridges and depths are most suitable for each area.
The Session
Conductive gel is applied. The transducer is moved over the treatment area, delivering shots at the programmed depth and energy. The doctor can alternate between different cartridges to work on multiple layers.
Duration varies: 40-60 minutes for face and neck, up to 60-90 minutes for large body areas (abdomen, thighs).
Discomfort During Treatment
HIFU produces a sensation of intense and brief heat with each shot. Tolerability varies among patients: some describe it as "uncomfortable but manageable"; others require oral analgesia pre-treatment. The doctor may recommend ibuprofen or paracetamol 30-45 minutes beforehand if sensitivity is high.
Post-Treatment
There is no real recovery time. There may be mild redness and some sensitivity in the area for 24-48 hours. Makeup can be applied the same day.
HIFU Results at Santé
- Immediate: mild-moderate tightening effect visible from the same day (contraction of existing collagen)
- 1-3 months: progressive improvement as new collagen forms and matures
- 3-6 months: optimal consolidated result
- Duration: 12-18 months, depending on the individual aging process
The recommended frequency is 1 session annually or 1 session every 18 months for maintenance.
Combinations to Enhance the Result
HIFU is enhanced by:
- Collagen biostimulators (Sculptra, Radiesse): HIFU acts on deep layers, biostimulators on the dermis — they are complementary
- Botulinum toxin: for dynamic wrinkles that HIFU does not address
- Skinbooster: improves surface skin quality while HIFU acts deeply
Frequently Asked Questions
How many sessions do I need?
One session is generally sufficient for the annual cycle. In cases of more advanced laxity, the doctor may recommend 2 sessions with an interval of 3-6 months.
Are results seen immediately?
There is a mild immediate tightening, but the main result appears between 2 and 6 months. The patient should not evaluate the result on the day of the session.
Can HIFU be done if I have Botox or fillers?
Yes, with appropriate intervals. The doctor determines the optimal sequence according to each patient's protocol.
How much does it cost?
The price varies depending on the areas treated and the number of cartridges used. We provide detailed information during the prior medical consultation.
