Biofillers: A Category of Fillers That Changes the Rules

Aesthetic medicine has worked for years with two distinct categories: fillers (hyaluronic acid, Radiesse) that add volume, and biostimulators (Sculptra, PRP) that stimulate tissue regeneration. What is PRF and why is it different from PRP

Many patients are familiar with PRP (Platelet-Rich Plasma), used for years in aesthetic medicine to treat hair loss, facial rejuvenation, and tissue repair. PRF (Platelet-Rich Fibrin) is an evolution of that same concept with important differences:

The PRF fibrin network is key: it acts as a biological scaffold that retains platelets and leukocytes in the tissue, and gradually releases growth factors over days and weeks, instead of all at once. The regenerative effect is more sustained and deeper.

How biofillers with PRF are prepared

  1. Blood extraction: 2-4 tubes without anticoagulant
  2. Immediate centrifugation: timing is critical — the fibrin polymerization process begins immediately and the working window is only a few minutes
  3. PRF separation: the fibrin fraction rich in platelets and leukocytes is obtained
  4. Biofiller preparation: liquid PRF (before its complete polymerization) can be used directly as an injectable, or combined with plasma gel to increase its viscosity and volumizing effect

The resulting product is a 100% autologous biological gel, rich in growth factors (VEGF, PDGF, TGF-β, IGF-1), with physical properties that allow its injection into areas of volume loss.

What biofillers with PRF do in the tissue

Once injected:

Immediate mechanical action

The gel occupies the space and visibly restores volume. The correction is immediate, although the gel eventually reabsorbs as the fibrin degrades.

Prolonged regenerative action

Fibrin retains growth factors in the tissue and progressively releases them over 2-4 weeks:

  • VEGF: stimulates the formation of new vessels (better tissue nutrition)
  • PDGF: activates fibroblasts to produce collagen and elastin

How long does the volumizing effect last?

Is it reversible?

Not directly. Being autologous, there is no enzyme that dissolves it. The gel degrades naturally, but it cannot be removed in case of an undesired result. Correct technique and indication are fundamental.
