Collagen Bio-stimulators: What They Are and How They Rejuvenate Skin from Within

What collagen bio-stimulators are, how they work, for whom they are indicated, and why they are the smartest alternative to progressive aging.

Collagen: what the skin loses over time

From the age of 25, collagen production in the skin begins to decline at a rate of approximately 1% per year. By 40, many people have already lost between 15% and 20% of the collagen they had at their peak. By 50, that figure can exceed 30%.

The consequences are visible: the skin loses density, deeper wrinkles appear, cheekbones flatten, the jawline loses definition, and the facial oval begins to sag. It's not just aesthetics: it's biology.

Collagen biostimulators are the most coherent medical response to this progressive loss. They don't disguise aging from the outside; they stimulate the body itself to produce new collagen.

What exactly is a collagen biostimulator?

A biostimulator is an injectable substance that, when placed in the subdermal tissue, activates fibroblasts—the cells responsible for producing collagen, elastin, and hyaluronic acid—to generate new extracellular matrix. The result is not an external gel that occupies space: it is the patient's own tissue, naturally generated over weeks and months.

Biostimulators act through an induced and controlled biological response. Depending on the product, the mechanism can be:

  • Controlled inflammatory (PLLA, as in Sculptra and Lanluma): microparticles trigger a localized fibroblastic response.
  • Biotic mineral (CaHA, as in Radiesse): calcium hydroxylapatite microspheres create a scaffold that guides collagen production.
  • Autologous (PRF/PRP/plasma gel): the patient's own plasma, concentrated with growth factors, stimulates tissue regeneration.

The main families of biostimulators

PLLA (poly-L-lactic acid)

This is the component of Sculptra (facial treatment) and Lanluma (large-volume body treatment). PLLA is a biodegradable polymer that the body progressively degrades while fibroblasts generate collagen in the area. The result: progressive and completely natural rejuvenation that lasts between 18 and 24 months.

CaHA (calcium hydroxylapatite)

This is the component of Radiesse. It has the particularity of exerting an immediate effect (the carrier gel acts as a temporary filler) in addition to the delayed biostimulatory effect. Widely used for jawline definition, hand rejuvenation, and neck treatment with a hyperdiluted technique.

PRF / Platelet-rich fibrin and plasma gel

Autologous biofillers are the most natural option of all: they are obtained from the patient's own blood, which is centrifuged to concentrate growth factors. PRF and plasma gel act as biostimulators and, in the case of plasma gel, also as viscoelastic fillers. Santé Clínica Estética is a pioneer in Barcelona in the use of plasma gel fillers.

Who are they indicated for?

Collagen biostimulators are especially suitable for:

  • Patients from 35-40 years old with visible volume loss, sagging, or deep wrinkles
  • People seeking a progressive and natural-looking result (without the "just done" effect of fillers)
  • Patients who want to improve overall tissue quality, not just correct a specific area
  • People with volume loss after significant weight loss
  • Patients looking for an alternative to aesthetic surgery

They are not the first option for young people who want an immediate volumizing effect (hyaluronic acid is more suitable there). They are the best option when the underlying problem is loss of structure, not volume.

Key differences with hyaluronic acid

| | Biostimulators | Hyaluronic acid |

|---|---|---|

| Result | Progressive (2-6 months) | Immediate |

| Mechanism | Own neocollagenesis | External filler |

| Duration | 18-24 months | 6-18 months (depending on area) |

| Appearance | Very natural | Natural if done well |

| Reversibility | Not directly | Yes (hyaluronidase) |

| Main indication | Loss of quality and structure | Specific volume loss |

Many patients benefit from a combination of both: hyaluronic acid for specific volume corrections and biostimulators to improve overall tissue quality. At Santé, we design combined protocols when medical evaluation justifies it.

The process at Santé Clínica Estética

Before any biostimulator treatment, we perform a complete medical evaluation: medical history, analysis of volume loss and tissue quality, reference photographs, and, if appropriate, skin analysis with advanced diagnostic equipment.

The treatment protocol is designed in a personalized way: which product or combination of products, in which areas, with what injection technique, how many sessions, and with what frequency.

Treatments are carried out at our clinic on Avenida Diagonal 384, Barcelona. The time per session varies between 30 and 60 minutes depending on the area and protocol. Recovery time is minimal: most patients resume their normal activities the same day.

Follow-up and maintenance

Biostimulators are not monthly maintenance treatments: they are tissue regeneration protocols. Once the initial cycle is completed (usually 2-3 sessions), the results are maintained for 18-24 months. From then on, an annual maintenance session is sufficient to sustain the results.

Our team performs systematic photographic follow-up and reviews progress at each session to adjust the protocol if necessary.

Frequently asked questions

When will I see results?

The first changes are noticeable within 4-8 weeks. The full result is consolidated between 3 and 6 months post-treatment.

Does it hurt?

Topical anesthetic is applied. Most patients describe the treatment as well-tolerated. There may be some sensitivity in the treated area for 24-48 hours.

How many sessions do I need?

It depends on the degree of tissue loss and the product used. Usually between 2 and 3 sessions, spaced 6-8 weeks apart.

Can it be done in summer?

Yes. Biostimulators have no seasonal contraindication. It is advisable to avoid intense sun exposure in the treated area during the first few days.

Can I combine biostimulators with other treatments?

Yes. Combined protocols with HIFU, radiofrequency, or hyaluronic acid are common and enhance the results of each technique.
