Smoker's Lines: Upper Lip Treatment

Vertical wrinkles above the upper lip, known as smoker's lines, are one of the most visible signs of facial aging. How they are treated at Santé Clinics.

Why they are called "smoker's lines"

Looking at a mouth with vertical lines above the upper lip is like reading a barcode: small, fine, parallel lines that run from the red border of the lip towards the base of the nose. It's such a recognizable image that it has become the colloquial name for a specific aging pattern: perioral wrinkles of the upper lip.

Although we refer to them by their aesthetic aspect, they have an important functional component: the perioral area is the region of the face with the highest muscle density per square centimeter, high daily mobility (speaking, smiling, drinking, smoking), and relatively thin, inelastic skin. Add accumulated sun damage and genetic predisposition, and the result is one of the areas that most quickly reveals age.

In this article, we explain exactly how smoker's lines form, what treatments address them, and how we combine techniques at Santé Clinics, in Barcelona, to achieve younger-looking lips without an artificial result.

How smoker's lines form

Three factors converge to form them:

1. Repeated muscle movement

The orbicularis oris muscle surrounds the mouth and contracts hundreds of times a day. This repeated contraction folds the overlying skin, just like a sheet of paper folded thousands of times eventually becomes creased.

2. Loss of structural support

With age, dermal collagen and elastin decrease, fat pads lose volume, and the labial mucosa thins. The skin of the upper lip loses "what supported it from within" and begins to collapse with muscle contractions.

3. Sun damage and habits

UV radiation accelerates elastosis. Smoking adds a specific muscle contraction (the puckering gesture) multiplied by thousands and, in addition, chronic vasoconstriction that damages the microcirculation of the area. The result is one of the most reliable signs of a smoker, even years after quitting tobacco.

Why a single treatment doesn't solve smoker's lines

The usual trap of the perioral area: wanting to solve it with a single technique. If the patient only receives neuromodulators, the wrinkles improve but the lip is left with loose skin. If they only receive fillers, they gain volume but the lines reappear when speaking. If they only receive laser, the skin improves but the muscle dynamics continue to fold it.

The correct approach to smoker's lines is combined. At Santé Clinics, we design protocols that integrate three or four different tools depending on the degree of aging, muscle pattern, and the patient's objective.

The tools in consultation

Perioral neuromodulators (micro-neuromodulators)

Neuromodulators partially relax the orbicularis oris muscle. Small, very well-distributed doses reduce contraction without affecting function (speaking, smiling, drinking). The effect is a decrease in the depth of dynamic lines and an improvement in the overall appearance of the lip.

  • Very low doses, precise technique.
  • Result in 7–14 days, duration 3–4 months.
  • Does not "eliminate" deep static wrinkles, but softens them.

Low-density hyaluronic acid

There are hyaluronic acids specifically designed for fine lines in delicate skin: low molecular weight, high integration. Applied with microbolus or microcannula technique directly into the line, they fill the wrinkle and restore volume to the red border of the lip without projecting the lip forward.

  • Natural results if the technique is correct.
  • Duration 6–9 months.
  • Risk of artificial appearance if the product or technique is not appropriate.

Biostimulants and skin boosters

Polynucleotides, low-dilution Sculptra, hyperdilute Radiesse, or specific skin boosters applied in the perioral area provide something that a filler does not: they regenerate the dermal matrix.

  • Stimulate type I and III collagen, elastin, and endogenous hyaluronic acid.
  • Cumulative result after 2–4 sessions.
  • Improves skin quality, not just volume.

Fractional laser

Fractional laser, especially in ablative variants (Erbium:YAG or CO₂) or powerful non-ablative ones, is the most effective tool for structurally renewing the skin of the upper lip. It creates micro-columns of regeneration that trigger neocollagenesis and soften static wrinkles.

  • Profound results, especially in patients with marked lines and sun damage.
  • Downtime of 5–10 days depending on technology.
  • Consolidated result in 3–6 months.

Chemical peels

Specific peels for the perioral area (25–35% TCA, phenol-croton in selected cases, medium-superficial retinoid peels) act on the epidermis and superficial dermis. Useful in an annual program and as a complement to other treatments.

At-home skincare

The daily routine is the foundation. Antioxidant Vitamin C, gradual retinoids according to tolerance, specific peptides for the lip contour, hydration with ceramides, and strict photoprotection are mandatory.

The combined protocol at Santé Clinics

A typical protocol for a patient with moderate smoker's lines:

  1. Session 1: assessment, photos, written plan. Initial skincare (Vitamin C, gradual retinoid, photoprotection). Perioral neuromodulators.
  2. Session 2 (3–4 weeks later): light hyaluronic acid in individual lines and red border if appropriate. Polynucleotides in the perioral area.
  3. Session 3 (4–6 weeks later): non-ablative or ablative fractional laser, depending on diagnosis.
  4. Session 4 (6–8 weeks later): second biostimulant application.
  5. Maintenance: neuromodulators every 4 months, bi-annual biostimulant maintenance, annual laser, daily skincare.

The plan is adapted to each case. In patients with very marked lines and advanced sun damage, we consider ablative laser (CO₂) as a central tool. In young patients with initial dynamic lines, neuromodulators and skin boosters may be sufficient.

Risks and considerations

  • Neuromodulators incorrectly dosed: loss of lip tone, difficulty sealing the mouth or pronouncing some consonants well. Dose and technique matter.
  • Fillers poorly chosen: artificial appearance, unwanted "prominent lip," nodules.
  • Ablative laser: post-inflammatory hyperpigmentation if preparation or phototype are not appropriate. Antiviral prophylaxis in patients with a history of herpes.
  • Too aggressive combination: respect for timing between techniques, do not accumulate laser and aggressive peels within a few weeks.

That is why smoker's lines are an area where the doctor's skill is very noticeable.

When will I see results? Neuromodulators: 7–14 days. Fillers: immediate (after swelling subsides, 5–7 days). Laser: 2–4 weeks. Biostimulants: 2–3 months.

How often do I need to repeat treatments? Neuromodulators every 4 months, biostimulants every 6–9 months, annual laser, daily skincare.

What if I smoke? The protocol works, but results last less. The honest clinical recommendation is still to quit smoking.
