Real volume, without surgery and reversible
Buttock augmentation with high-density hyaluronic acid is, today, the closest non-surgical alternative to the effect of a classic buttock augmentation. It provides visible volume from the first session, improves contour and, unlike an implant or lipotransfer, is reversible: if the result is not satisfactory, it can be degraded.
It is one of Santé's most requested treatments and also one of the most technically demanding. That is why it is performed exclusively by two of our specialists: Sofía and Nacho Paradelo. In this article, we explain what it is, how it works, how much is used, what realistic results are, and what the limitations are.
What is high-density hyaluronic acid
Hyaluronic acid is a molecule that the body naturally produces, present in skin, joints, and the extracellular matrix. Injectable formulations are cross-linked gel of biotechnological, non-animal origin hyaluronic acid.
For buttocks, specific high-density and high-projection capacity vials are used — different from facial products — designed to be injected into the deep subcutaneous plane and to withstand the mechanical pressure of the area.
Key features:
- Reabsorbable: the body slowly degrades it.
- Reversible: if necessary, it can be dissolved with hyaluronidase.
- Hydrophilic: attracts water, which provides volume and elasticity to the tissue.
- Biocompatible: with a very high safety profile when applied by a qualified doctor with certified product.
What it does and what it doesn't do
It does:
- Real volume increase of the buttock.
- Improved contour: rounding, projection, correction of “hip dips” (lateral depression).
- Elevation of the upper band, giving the sensation of a higher buttock.
- Natural result in expert hands: without looking “operated”.
It does not:
- It does not correct severe ptosis (sagging). For that, collagen stimulation or surgery is needed.
- It is not a surgical BBL. The reasonable maximum volume is limited by the plane and the amount of product that can be safely placed.
- It is not forever. The average duration is 18–24 months.
- It does not replace the gym. A well-worked buttock responds better.
How much is used
The amount depends on anatomy and objective, but in the buttocks, it is always much greater than in the face. It is measured in 10 to 20 ml vials, not in 1 ml syringes. Typical protocols at Santé range from 20 to 60 ml per session (10–30 ml per buttock), distributed in one or two sessions weeks apart.
Larger volumes are assessed in sequential sessions to ensure safety and a harmonious result.
How it's done, step by step
- Consultation and assessment with Sofía or Nacho Paradelo. Photographic study, palpation, planning.
- Marking of projection areas: superolateral quadrant, inferior quadrant, correction of hip dips if applicable.
- Topical and local anesthesia at the entry points.
- Injection with a high-gauge blunt cannula, in the deep subcutaneous plane, in a fan pattern, avoiding vascular structures and the deep muscular plane. The latter is essential: buttock hyaluronic acid is applied subcutaneously, not intramuscularly.
- Manual modeling of the product.
- Post-treatment recommendations.
Duration: 60–90 minutes. It is an outpatient procedure, and you can walk out.
Post-procedure and recovery
- 24–48 hours: slight inflammation and sensitivity. Possible bruising at entry points.
- 3–7 days: inflammation subsides. You can return to your routine.
- Recommendations: avoid sleeping on your stomach and prolonged sitting for the first 48 hours, avoid intense buttock training for 7 days, hydrate well.
- Final result: stabilized at 2–4 weeks, when initial inflammation subsides and the product integrates.
Who is an ideal candidate
- Women (and men) seeking to improve buttock volume and contour without surgery.
- Patients with small or flattened buttocks and skin with good tone.
- Patients with pronounced hip dips that break the lateral line.
- Those who value reversibility and prefer to start with something modulable before considering surgery.
Who is not a candidate
- Pregnancy or lactation.
- Active local or systemic infection.
- History of reaction to similar products.
- Severe flaccidity: in that case, collagen stimulators or surgery are more appropriate.
- Expectation of a greater anatomical change than the technique can offer.
Risks and safety
Buttock hyaluronic acid, in qualified hands and with certified product, has a very high safety profile. Risks to be aware of:
- Hematoma and local inflammation (frequent, self-limiting).
- Correctable asymmetries.
- Granulomas are very rare, manageable.
- Vascular risk: requires cannula technique and anatomical knowledge. That is why choosing an experienced professional is crucial.
- Inadequate or counterfeit product: the main risk in the market. At Santé, we work exclusively with certified and traceable brands.
What about lipotransfer or classic BBL?
Surgical BBL offers greater volumes and uses one's own fat. It is a major surgery with significant risks and a long recovery. Buttock augmentation with hyaluronic acid does not compete with that; it offers a different path: outpatient, reversible, with a more subtle result. The patient decides what they want.
Smart combinations
- Collagen stimulators (Sculptra/Radiesse) + hyaluronic acid: stimulators tighten, hyaluronic acid adds projection. A benchmark combination for natural and firm buttocks.
- Body HIFU + hyaluronic acid: HIFU for flaccidity, hyaluronic acid for volume.
- Body mesotherapy + gym protocol: the piece that most changes the long-term result.
Frequently asked questions
Is it safe to inject so much product? It is when the injection plane is correct (subcutaneous, not deep muscular), the product is certified, and the quantity is consistent with the anatomy. That is why only two specialists at Santé perform it.
Can it be undone if I don't like it? Yes. There is an enzyme (hyaluronidase) that dissolves the product if necessary.
Does it hurt? It is tolerable. Topical + local anesthesia. A sensation of pressure rather than pain.
How long does it last? 18–24 months depending on the patient, skin quality, and physical activity.
When are the results seen? Immediate, definitive at 2–4 weeks when inflammation subsides.
Can I combine it with surgery later? Yes, without problem. Hyaluronic acid does not contaminate the plane for future surgery.

Buttock augmentation with hyaluronic acid is not a “first consultation and done” treatment. We need to see you, discuss objectives, review reference photos, plan volume and plane, and, above all, be honest with you about what can and cannot be achieved.
