Why they are called “barcode lines”
Looking at a mouth with vertical lines above the upper lip is like reading a barcode: small, fine, parallel lines that extend from the red border of the lip towards the base of the nose. It's such a recognizable image that it has become the colloquial name for a specific aging pattern: perioral wrinkles of the upper lip.
Although we refer to them for their aesthetic aspect, they have an important functional component: the perioral area is the region of the face with the highest muscle density per square centimeter, high daily mobility (speaking, smiling, drinking, smoking), and relatively thin, inelastic skin. Add accumulated sun damage and genetic predisposition, and the result is one of the areas that most quickly reveals age.

How barcode lines form
Three factors converge to form them:
1. Repeated muscle movement
The orbicularis oris muscle surrounds the mouth and contracts hundreds of times a day. This repeated contraction folds the overlying skin, just as a sheet of paper folded thousands of times ends up creased.
2. Loss of structural support
With age, dermal collagen and elastin decrease, fat pads lose volume, and the labial mucosa thins. The skin of the upper lip loses “what supported it from within” and begins to collapse due to muscle contractions.
3. Sun damage and habits
UV radiation accelerates elastosis. Smoking adds a specific muscle contraction (the pursing of the lips) multiplied by thousands and, in addition, chronic vasoconstriction that damages the microcirculation of the area. The result is one of the most reliable signs of a smoker, even years after quitting tobacco.
Why a single treatment doesn't solve barcode lines
The usual pitfall of the perioral area: trying to solve it with a single technique. If the patient only receives toxin, the wrinkles improve but the lip is left with loose skin. If they only receive fillers, they gain volume but the lines reappear when speaking. If they only receive laser, the skin improves but the muscle dynamics continue to fold it.

Biostimulants and skinboosters

Fractional laser

Risks and considerations
- Toxin incorrectly dosed: loss of lip tone, difficulty sealing the mouth or pronouncing some consonants well. Dose and technique matter.
- Fillers poorly chosen: artificial appearance, unwanted “prominent lip,” nodules.
- Ablative laser: post-inflammatory hyperpigmentation if preparation or phototype are not appropriate. Antiviral prophylaxis in patients with a history of herpes.
- Too aggressive combination: respect for timing between techniques, do not accumulate laser and aggressive peels within a few weeks.
